TPO - The results of the US Navy's investigation into the submarine's collision with an underground structure under the South China Sea in October last year were a "preventable" incident, caused by errors in planning and risk management.

Eleven sailors suffered minor injuries in this incident, the most serious being scalp lacerations and broken collarbones. But on the way back to the base in Guam, the ship's commander determined that more than a third of the crew needed mental health treatment.
