This error should not have occurred, cos the nurses are experienced and well trained personnel...

Already told many..our top notch healthcare system are not really that superior. Outside very nice inside messy like economic rice stall...

Given that the front line nurses are already under stress for so long, I can't bear to blame them for the mistake.

But, those working in the backend office, did they play as a team to remind front line folks about change of syringe?

Or my job is just to do procurements, front line you need to open eyes big big..
