They just subscribe to the Voip service. Some free. Voip can be use on desktop, laptop, smart phone, etc. The caller phone number and country code can be inserted in. Auto-recorded program like auto-dial fax machine. Similar to Skype.

A lot of the calls to me are with prefix numbers. Even for local calls and SMSs, they have prefix numbers. Likely, they use Voip or pre-paid cards. The SMSs from clinic, courier company, utilities company, property agents and even government bodies have prefix numbers.
