Maybe they infected by other mild virus which

A new study assessed the dynamics of the replication of the new coronavirus, SARS-CoV-2, in the upper respiratory tract and the body’s early immune response.
The study reports that SARS-CoV-2 replicates exponentially immediately after infection and results in an immune response that lags by a few days.
The results suggest that an early and robust immune response may inhibit viral replication.
Pre-infection with a different respiratory tract virus — such as a cold virus — inhibited SARS-CoV-2 replication, suggesting that interactions between viruses may influence their transmission.

In the first place, below 50years old death are rare unless with underlying conditions.
Africa median age is young.

Hard for SARS-CoV-2 to multiple as there are many other virus or disease competing.

The infection data is definitely not accurate for obvious reasons such as the lack of test kits. The more reliable data would be the surprisingly low death figures. Dat could be due to factors such as few international tourists especially Delta virus carrying ones travel there. And the locals there have a beri resilient immunity system from being exposed to all kinds of pre-existing viruses and diseases.

No test no infected is true, lack of tourists is also true.
Less dense areas are more, less air pollution, lack of transport etc limit spreading.

Death is low not unexpected as Africa population is young.
While darker skin limit vitamin D absorption, they are exposed under the sun longer 
With number of other viruses compete with covid for multiplying.
