Oh SH!T...Air India 🇮🇳 crash Report shows pilot confusion

OMG. This 1st official investigation report does not look good for Indian pilots..

Flight AI171 was flown by Captain Sumeet Sabharwal, 56, who had a total flying experience of 15,638 hours and, according to the Indian government, was also an Air India instructor. and co-pilot was Clive Kunder, 32, who had 3,403 hours of total flying experience. The fuel switches had almost simultaneously flipped from ‘run’ to ‘cutoff’ just after takeoff. The preliminary report did not say how the switches could have flipped to the cutoff position during the flight. Experts said a pilot would not be able to accidentally move the fuel switches.

I hate to come a simple conclusion - that fuel-cut off was a DELIBERATE ACT by one of the pilot just as the 787 lift off. There is no way the engines can spool up quickly back to full power at such attitude and low altitude... This morning Report from India investigators indicates BOTH engines fuel were cutoff and toggled back on again by one of the pilots.

Preliminary report summary -

  1. When the 787 achieved its top recorded speed of 180kts (3 sec after takeoff) , "the Engine 1 and Engine 2 fuel cutoff switches transitioned from RUN to CUTOFF position one after another with a time gap of 01 sec".
  2. One pilot asked the other why he cut off fuel, and the second pilot responded that he had not.
  3. The aircraft quickly began to lose altitude.
  4. The report does not indicate there was any emergency requiring an engine cutoff.
  5. The Indian bureau said the investigation was ongoing...

There are a series of switches the pilot must switch on or off during take off and landing. One of the pilot must have inadvertently "off" the fuel supply instead of the intended switch.      Confused

Takeoff main actions are gear up, flaps up while keeping throttle at takeoff power.

NEVER ever touch anything else while still in critical flight phase like taking off... This includes fuel related controls...which is ONLY needed when shutting down the engines at the gate after landing.

The Fuel-Control has a safety feature which requires PULLING before changing the switch position, this helps prevent accidental toggle...

could it be a kind of cultural behaviour, talking, talking and talking and forgetting where they were and what they should be doing.
just hope that such incidents don’t happened locally here as we already have imported such a massive labour force into our country, including such pilots for scoot.😰

This is known as "sterile cockpit discipline" -  Set of piloting rules aimed at reducing distractions during critical phases of flight, specifically during taxi, takeoff, landing.

for me, it was such an inconsiderate behaviour,  and worst of all, speak at the top of their voice and most of the times they use speaker mode. it is really disgusting, especially in enclosed areas, like MRT or buses.

Indian Pilots Association criticised the Preliminary Report for insinuating the crash may be due to pilot error.
Anyone surprised ?  Big Grin

The Airline Pilots’ Association of India (ALPA-I) has raised serious concerns about the preliminary probe into the Air India AI171 crash ALPA-I further rejected any suggestion that the accident was due to pilot error, stating, “The tone and direction of the investigation suggest a bias toward pilot error. We categorically reject this presumption and insist on a fair, fact-based inquiry.”

The flight recorder sud also include video of the cockpit

Nowadays video recording is fcuking cheap and super high res and storage can fifo, so it is a non issue

looks like in future pilots may have to go thru some test just before flight start.
like is pilot drunk?, pilot got emotional problem etc.

If u asked me, why cant video and blackbox data be live transmission back to control center ?

This is modern day liao....and with AI era coming. This is important!!
