Taipei, March 28 (CNA) Bongkrek acid was the likely cause of a deadly food poisoning outbreak at Malaysian restaurant chain Polam Kopitiam in Taipei, according to health officials. Outbreak has killed at least 2-people, 12 others who dined at Xinyi District have received medical attention, Health Minister Hsueh Jui-yuan (薛瑞元) told reported. On Wed evening, the Deputy Health Minister Victor Wang (王必勝) said sickened diners were likely to have ingested Bongkrek acid, maybe found in rice products contaminated by rare bacterium Burkholderia gladioli during fermentation.

This was reached by expert panel of toxicology & infectious disease experts, representatives from Centers for Disease Control (CDC), Taiwan Food & Drug Administration (TFDA), hospitals treating the affected patients, & the Institute of Forensic Medicine, Wang said.

How food @Polam Kopitiam became contaminated is under investigation, Hsueh said deliberate intervention by human has not been ruled out, sickened individuals all dined at Xinyi branch of Polam Kopitiam between March 19 and 22, consumed dishes such as flat rice noodles & kway teow (a kind of rice noodles) made from same ingredients.

Clinically, especially among those with severe symptoms requiring hospitalization, the symptoms were similar, with acute liver failure leading to subsequent organ failure, Wang said. Toxicologist Yen Tzung-Hai (顏宗海), was part of expert panel, told CNA.

Bongkrek acid was first discovered in Indonesia, where several food poisoning cases occurred due to consumption of a traditional coconut-fermented cake called Tempe bongkrek.

One milligram of Bongkrek acid can be fatal, Yen said, adding that the toxin directly damages the mitochondria responsible for energy production in human cells, thus affecting cellular respiration. Symptoms resembling acute gastroenteritis, such as nausea, vomiting, & diarrhea will develop after ingestion of food contaminated with Bongkrek acid, with possibility of multiple organ failure within one day and a mortality rate exceeding 40% Yen said.

In terms of treatment, Yen stated that there is no antidote for Bongkrek acid, with only supportive therapies available, such as intubation for respiratory failure or administering inotropic agents for shock, while, Wang said that an investigation is underway, with the TFDA having collected specimens from patients and 35 food samples from the restaurant for testing, while the TFDA, National Taiwan University Hospital, and Taipei Veterans General Hospital have placed orders for standard samples required for testing Bongkrek acid.

While the testing is expected to commence next week, Wang pointed out that the forensic autopsy conducted on Thursday will include tests for pesticides and other toxins, rather than solely focusing on one substance.

If the food poisoning outbreak is confirmed to have been caused by Bongkrek acid, it would be Taiwan's first case of collective food poisoning caused by Bongkrek acid, Wang added.

Taipei, March 28 (CNA) Bongkrekic acid, a fatal toxin, was found in the blood of one of the deceased in the Taipei food poisoning outbreak after forensic examination, Deputy Health Minister Victor Wang (王必勝) said Thursday.

At a press conference on Thursday evening, Wang announced that the results of the forensic examination conducted by experts from National Taiwan University's Department of Forensic Medicine revealed the presence of a rare toxin, Bongkrekic acid, in the blood of one of the deceased, marking the first ever detection of the toxin in Taiwan.

Wang stated that it aligned with the conclusion of the expert panel held on Wednesday, but he reiterated that this is not the final conclusion, emphasizing that the authorities chose to reveal the results of the forensic examination because "the public is very concerned."

"The final conclusion will be made after a comprehensive assessment by prosecutors," he added.

As of 5:00 p.m. Thursday, 18 people had sought medical attention in the greater Taipei area and Tainan after eating at a branch of the Malaysian restaurant chain Polam Kopitiam in Taipei's Xinyi District, two of whom died on March 24 and March 27, according to information from the Ministry of Health and Welfare.

Chiang Chih-kang (姜至剛), a professor at the Graduate Institute of Toxicology at National Taiwan University's College of Medicine, said that Bongkrekic acid mainly affects the liver, kidneys, and brain, and death can occur within 20 hours after the onset of symptoms.

According to toxicologist Yen Tzung-Hai (顏宗海), Bongkrekic acid was first discovered in Indonesia, where several food poisoning cases occurred due to the consumption of a traditional coconut-fermented cake called Tempe bongkrek.
