Something is crawling inside it. Sometimes the head can appear on either end. At first I thought it was a deserted empty nest or shell of sort. Then I saw a head appeared at one end and then went back inside and re-appear at the other end. What pest is that?

I guess they should be clothes moths. I hate these insects as their larvae can make a horrible mess if they ended up in your drawers or clothings.
I don't use insect spray so can only catch them (adults) using those "buttefly" catching net from Daiso and then spray alcohol to immobilise them (water doesn't work as the wing is waxy). Then dump them in the toilet and light them up (using a stove lighter) to kill them permanently.

Much easier to net them when 2 adults are mating (their back sticking to each other) and thus less likely to fly off:

My domestic helper tells us that they are worms? Usually hang on walls but will cause extreme itchiness when your skin comes in contact with them. She normally takes a broom and vigorously dusts them off.
Not too sure about hygiene as all the dirt from the ceiling will fall them on all appliances, kettles, utensils. So will wash everything after that. Not that bright in the brains department but hey, I am not in charge of the domestic department

That is cocoon stage of some kind of small insects. Soon some small insects will fly out of those cocoons to bite or entertain the residents in that room.......

I just use masking tape to stick them and throw away. I have been seeing these for some time now. Not many, but still irritating as they will destroy our clothes or paper products.

instead of spraying insecticides or removing them manually, can use yellow stickers or insect zappers to catch many of the flying adults

Agreed.
That is the only way to suck them off from the ceiling.

Yup! This is exactly what I saw. The wriggling larvae sticking out their head on either end. So it is the larvae of moth.
I have gather them manually via wet toilet tissue and flushed them down the toilet.
Now the ceiling and wall looks cleaner. Its an eyesore when too many up there.
