We really cannot tell from external appearances. Most peepur appear healthy on the surface of things, but they may not really be healthy at all. A full body checkup will reveal a lot of things.
Peepur who suffered a stroke did not appear to be sick, limp about, or had no appetite, etc etc. Talking about appetite, they may even be tucking in on Jollibee or Texas Fried Chicken, without a care in the world, and we know these fried stuff are bad for heart health. (Same goes for Oil Fried Ghost, satay, char kway teow...)
They can talk, joke with you, drive their cars, sing, just like anybody else, looking so normal and healthy, and the next minute, they feel weak on one side of the body, their face droops...
It's scary, isn't it? Imagine reading and responding to a SgTalk post, smoking and a glass of iced cold beer by the side, song song gao jurong, and suddenly, you feel weak or get into a coma. It happened to Heng Swee Keat but he was lucky that the horsepeter was not far from Parliament House, Singapore being a small country anyway, and he received the earliest medical intervention possible. He had an aneurysm.
