I heard the Indians tried to push the blame to the pilot even when the International maritime highway code states that nothing will absolved the captain's responsibility even when the ship is under pilotage.

Dont laugh!
The questions to ask.
How come the bridge is so important, yet there is no safety barrier to protect the bridge from hitting.
How come 2 guide boats MIA and never guide the ship all the way.
Only 4 mins reaction time, 800 to 900m away from the crash site. The sky was dark at that time.
What would be your first action?
To stop the ship right.
